How to Refer a Patient for Follow-Up Medication Adherence Services

The ScriptAssist telephonic nurse-outreach support service is absolutely free for patients whom you refer. The purpose of this service is to promote patients’ adherence to prescribed treatment, while enhancing the physician-patient relationship. This service is being made available free to physicians and their patients, through pharmaceutical company support. In order to refer a patient:

  1. Tell your patients: “I’m working with a nursing support service called ScriptAssist. I’d like to have them call to see how you are doing, and to help in answering any further questions you have during the course of your treatment.”
  2. ScriptAssist provides a consent form for you or your office staff to use in enrolling patients.
  3. Please send ScriptAssist the following information about the patient whom you are referring for follow-up:
    • Patient’s Name
    • Patient’s Telephone Number
    • Prescription Information (Prescription Date, Dosage)
    • Best Times to Reach the Patient by Phone (if known)
    • Whether or Not We Can Leave a Message (if known)
